The Treatment Desert Crisis: 3 states โ€” Texas, Puerto Rico, South Carolina โ€” have fewer than 2 treatment facilities per 100,000 residents. Texas, the second-most populous state in the nation with nearly 30 million people, has just 453 facilities โ€” a rate of only 1.55 per 100,000 residents.

The Access Divide: 28 additional states fall in the "Underserved" category (2โ€“5 facilities per 100k), meaning 31 out of 52 states and territories lack adequate treatment infrastructure.

The Best-Served States: Maine leads the nation with 11.08 facilities per 100,000 residents, followed by Alaska (10.64), Kentucky (10.05), Wyoming (9.01), and North Dakota (8.73). Notably, these tend to be smaller states where state-funded programs play a larger role.
Methodology: This analysis examined 13,460 alcohol and substance use treatment facilities listed on Alcoholism.org as of April 23, 2026. Facilities per capita were calculated using 2024 U.S. Census Bureau population estimates. A "Rehab Desert" is defined as a state with fewer than 2 treatment facilities per 100,000 residents โ€” a threshold aligned with public health research on minimum care infrastructure. This analysis covers SAMHSA-registered facilities offering alcohol treatment, detox, or co-occurring disorder services.
